Pursuant to Article B, Section 3B.01 of Tax Ordinance 2009-002 series of 2009, Persons Governed. You can get your individual work permit from the Cebu City Hall. It is issued by the Cash Division. An Individual Work Permit is required for workers or employees, whether temporary or permanent, working with the jurisdiction of the City Mayor.
In a lot of instances, it is also called as Occupational Permit, which you will get from the City Mayor. Please do not get confuse with the Individual Work Permit. Just tell the person you are trying to get an Occupational Permit, which is an Individual Work Permit and then, mention your company.
Most call centers require their employees to get the Individual Work Permit. You have to go the City Hall and start your application early so you can complete the entire process in one day. It is considered unlawful for any person to exercise his occupation or calling within the jurisdiction of the city, without acquiring the Individual Mayor’s Permit.
Step 1. Before applying for the Occupational Permit, you must first secure your Health Card from the City Health Department, especially if you are applying the Occupational Permit for the first time. Apply for the Occupational Permit only after you get your Health Card.
Step 2. Get the form, Occupational Permit Information Sheet at the Business Permits and Licensing Department. Attach and submit the following requirements along with your duly accomplished, filled up information sheet.
Community Tax Certificate
Police Clearance or NBI Clearance
Health Card and Health Official Receipt
Yellow Cards (for entertainers and therapist)
Waiver for Professional Regulatory Commission (PRC) cardholder
Parent’s Consent (if minor)
Alien Employment Certificate (if foreigner)
Department of Public Order and Safety Clearance (for security guards)
Need proof of payment
Entertainer Health Certificate – GRO
Sputum Test – Tricycle Driver
Birth Certificate – Minors
Endorsement Cert from DSWDO-GRO’s and working minors
Certificate of Seminar/ Orientation from CVO – Butchers
List of employees for group applicants – Corp/ Inc (board/ non board passers)
Old electronic ID – renewal
Step 3. Have your picture taken at the Business Permit Licensing Office.
Step 4. Wait for the release of your occupational permit.

Who may avail of the service?
Employees or workers in generally considered offensive and dangerous business establishments.
Employees and workers in commercial establishments who generally enter or attend to the daily needs of the general public such as but not limited to the following: employees and workers in drugstores; department stores; groceries, supermarkets; beauty salons; barber shops; tailor shops; dress shops; bank teller; receptionist, receiving clerk in paying outlets of public utilities corporation, except transportation companies; and other commercial establishments whose employees and workers attend to the daily needs of the inquiring or paying public.
Employees and workers in food or eatery establishments.
Employees or workers in night or night and day establishments.
Workers or employees in bars; boxing stadium; bowling alleys; billiards and pool halls; cinema houses; cabarets and dance halls; cocktail lounges; circuses; carnivals and the like; day clubs and night clubs; golf clubs; massage clinics, sauna baths or similar establishments; hotels; motels; gym and sports establishments; private detective or watchman security agencies; supper clubs and all other business establishments whose business activities are performed and consumed during night time.
All other employees and persons who exercise their profession, occupation or calling within the jurisdictional limits of the city subject to periodic inspection, surveillance and/or regulations by the City Mayor.
The Individual Work Permit was enacted to regulate the practice of certain occupations that, by their nature, present a heightened public safety risk to the public.
